Download our simple claim form. It's a PDF file, so you'll need Adobe Acrobat Reader to open it.
Type your information directly into the form and click the Print Form button in the top right corner, or print the form and fill it in by hand. Make sure you sign it at the bottom.
Fax, mail, or scan and email the form along with an itemized invoice from your veterinarian to us. Our contact information is right at the top of the form. Remember to submit your claim within 180 days of the date of service.

You can track the status of your claims online at our free Member Center. There you can also update your contact information, change your payment method, select our "Go Paperless!" option, and more. You'll need to take a moment to sign up if you haven't already.
