Claim Form
It's easy to submit a claim:

Download our simple
claim form. It's a PDF file, so you'll need Adobe Acrobat Reader to open it.

Fill out the form making sure both you and your veterinarian (or an authorized representative) sign it at the bottom.

Fax, mail, or scan and email the form along with an itemized invoice from your veterinarian to us. Our contact information is right at the top of the form. Please submit your claim within 180 days of the date of service.
